MCTG | Cover up and perjury

My son died in 2017, at his inquest later that year the trust witnesses lied to the Coroner, they withheld crucial records, pages 3-5 and pages 77-82 of the care pathway. Pages 3-5 included the index for the pathway but also included details of admission and discharge, pages 76-82 was the discharge section. At inquest the coroner issued a regulation 28 based on the trust not having a policy for sharing info with family members on discharge. This was a deception as the trust did have a policy for sharing info, the coroner didn’t bother to find out.

When my son was in crisis I attended a scheduled appointment with his consultant, he had been discharged from A&E not 8 hours earlier after overdosing, when discharged he took an additional overdose, there is too much information to impart so I refer to bear and scully podcast, “did the nhs cover up my sons death” every regulator and the ombudsman were involved in the cover up, we commissioned an independent expert who stated unequivocal negligence.
